Bulbous Nose Deformity

The word bulbous itself clarifies that nose tip is more prominent, bigger than normal, not suiting to the rest of the nose or face.

This is one of the most common complaints we hear from clients seeking nose job/ rhinoplasty.

  • What is the cause of this excessively prominent nose tip?
  • How do we tackle this shape abnormality?
  • Is it really possible to make the nose tip so thin and narrow?

I am going to answer and explain regarding above questions

Nose tip has 3 important components: skin, fat and cartilage in layers from surface to the deep.

It is logical to say excess of the any of the above components in any dimension will produce more prominence. That means excess skin thickness, excess fat under the skin and excess cartilage may be the basic cause. All three components may not be the problem in all given individuals. It maybe the isolated excess of single component: for example too thick skin, surplus fat and wide cartilage.

We de-glove the skin, remove the fat underneath, thin the skin and trim the excess cartilage and reorient them in to proper position; simply these technical steps executed carefully will narrow your bulbous nose tip.

Yes, it is possible to make nose tip narrow to significant extent, among the above mentioned three components fat under the skin and cartilage correction is possible only limiting factor is thinning the skin in very thick skinned individuals. Have sensible expectations, it is almost impossible to achieve sharp tips of Caucasian (white skinned) from Indian ethnic origin noses.

There are no adverse effects on the functions as well as the structure of the nose tip as long as surgeon executing all the technical steps vigilantly but occasionally patients skin factors and others may also create a problem.

You will perceive satisfactory results after 3-4weeks but it will continue to improve till 3,4,6 months. So patiently wait and watch for the results. It’s actually good you don’t have sudden surprises and it’s easy to adapt for a change slowly.

Its 1-2hrs procedure, need not stay overnight in the clinic, performed in painless anesthesia, minimal discomfort after the surgery, and could be easily managed with tablets for 5 days. Actually there is no downtime but practically there is going to be dressing cover 5-6 days.

Crooked Nose Deformity, Deviated Noses

Crooked nose is also known as deviated, bent, curved or twisted nose: means describing the shape of the nose.

It is one of the cosmetic or appearance concerns of the nose for many people. Some people with this problem may also complain difficulty in breathing through the nasal openings and frequent cold etc

  1. Why noses are deviated or crooked for some people?
  2. Does this problem produce some nasal functional troubles?
  3. Do rhinoplasty/ nosejob of these noses correct/ fix the problem?

  5. What if they have already undergone surgery but still the problem of deviation exists?

Following are the detailed answers for the above questions

Causes of the crooked nose deformity are fall or injuries which are noticed some times unnoticed during childhood, surgery on the nose in the childhood i.e. before 14 years of age or just mere growth related or developmental without any surgery or injury. Sometimes the elders in their family may have similar noses.

Most of the people with deviated noses may have frequent cold and nasal blockade; the main reason for frequent cold is deviated nasal septum which commonly associated with twisted noses. Nasal septum is a structural wall that separates nasal cavity in to right and left sides, it is usually a midline structure but sometimes it is bent like C or S causing both functional and shape related troubles.

Crooked nose correction is nothing but bringing the nasal pyramid structure midline by the surgery. The surgery involves straightening of the nasal septum and other bones in to midline and same time it improves the breathing as well as shape.

Only correcting the septum i.eseptoplasty surgery by ENT specialist won’t solve the problem completely perhaps it makes the condition more difficult to correct the external shape. The complete procedure septo-rhinoplasty involves correction of both nasal septum and external nose shape in one stage. It is 2-3 hrssurgery, performed under anaesthesia, nose will be opened from inside both nasal septal cartilage and bones will be exposed and straightened. Nasal tampon is inserted inside the nose after surgery and external plaster cast is put. A day in the hospital and one week at home is mandatory. One can walk around and perform basic activities even work from home on computer after 48 hrs after surgery. Antibiotics and pain reliving medication will be prescribed for one week.

No need to use long term medication.

Person with deviated nose had septoplasty in the past but again seeking the external nasal shape correction, are called as revision/redo cases. It is possible to do redo correction but they need a septo-rhinoplasty which is going to be more technically demanding, reason being part of the septum is corrected and there is a cartilage loss in the previous procedure. In these cases we may need cartilage material from other parts of the body to build the nasal structure. I advise you to proceed with the internal and external work same time by a person who is well versed in performing both means the functional and structural aspects of the nose.
